Valid FAA Class 1 Medical Certificate

Proof of Insurance (Current Policy)

Valid Driver's License

Recent Chest X-Ray (within 12 months)

Recent EKG (within 12 months)

Morning-of Preparation

Ensure you are well-rested and hydrated. Avoid caffeine and alcohol for at least 12 hours prior to the exam. Bring a pen and notepad for your medical history review.

Important: Please arrive 15 minutes early to complete the intake form and check-in.
